Verification is required for all driving records ordered for Personal Use. Please select one of the following verification options .

OPTION 1: TEXT, FAX or EMAIL a legible copy of your driver's license or a State/Government issued photo ID

OPTION 2:TEXT, FAX or EMAIL a signed release* (click here) and ONE of the following items:

1) A legible copy of your vehicle insurance card with your name and address on it.
2) A legible copy of your vehicle registration with your name and address on it.
3) A VOIDED check that has your full name and address on the check.
